📋 ASSESSMENT TERMS & CONDITIONS
Consultation Reservation
Purchasing this assessment reserves a time slot with Restorative Therapeutics Clinic (Medical Massage Professionals, LLC). It does not guarantee an exact appointment time. A timeframe will be coordinated after the Authorization for Consultation form is completed.
🚫 Non-Refundable Fee
All assessment fees are strictly non-refundable.
Once payment is completed, the assessment process begins immediately and cannot be reversed. This includes, but is not limited to:
- Change of mind
- Financial reconsideration
- Scheduling conflicts
- Failure to attend or connect
- Decision not to proceed with treatment
- Email delivery issues on the client’s side
By completing this purchase, the client acknowledges that the assessment is a standalone professional service and agrees that no refunds, reversals, or chargebacks will be issued under any circumstances.
📝 Authorization Form Required (Florida HB 197)
Evaluation scheduling is conditional upon completion of the Authorization for Consultation form. This form collects surgical history, areas of concern, and valid photo identification (required by Florida law, House Bill 197).
🔍 Service Description
The evaluation includes:
- Review of surgical and post-operative history
- Observation of areas of concern
- Internal preparation of a Report of Findings
- Determination of candidate suitability
No treatment, protocol, instructions, or independent recommendations are provided at this stage.
The consultation service is considered rendered after the clinical assessment and preparation of the Report of Findings, regardless of whether the client chooses to open, read, or act upon the report.
📅 Scheduling & Office Hours
- Assessments are scheduled Tuesday–Friday, 8:00 AM–4:00 PM (EST)
- Remote consultations may be rescheduled up to three times; in-person assessments require 48 hours’ notice for changes
- Candidates are responsible for checking time zone differences
📧 Electronic Delivery & Client Responsibility
All consultation reports and communications are delivered electronically via email to the address provided at checkout.
Clients are solely responsible for:
- Providing a valid and accessible email address
- Checking spam, junk, promotions, or filtered folders
- Whitelisting our domain prior to purchase if using legacy or restricted email providers (e.g., AOL, Yahoo)
Failure to receive an email due to filtering, inbox limitations, or client-side email settings does not constitute non-delivery and does not qualify for a refund or chargeback.
